Use the General Demographic Request to change your personal information, such as name, address, telephone numbers, personal and business e-mail addresses, and so on. Demographic changes to an employee's address will affect only the employee's primary address on file in Alio (i.e., the Address Type equals PR). If an employee has multiple addresses on file in Alio, just the primary address is updated.
If allowed by your district, you can:
Elect to receive a printed Advice of Deposit document or an electronic Advice of Deposit document that can be printed. This field displays only if the SHOW_DIRECT_DLINQ EPOSIT_OPTIONS profile key is set to Y in Alio and the Direct Deposit Print option is selected by the administrator.
Submit a request to receive a printed W-2 Form from your district or an electronic W-2 Form from your district that you can print. This option displays only when the W2 Print option is implemented by your LINQ EP system administration.
Submit a request to receive a printed 1095C Form from your district or an electronic 1095C Form from your district that you can print. This option displays only when the ACA Print option is implemented by your LINQ EP system administration.
If the employee is linked to a vendor record, the vendor address is automatically updated with the same change.
To submit a General Demographic request:
Click the Request Changes link on the Home page. The Demographic Requests page displays.
Click the Request
Type Drop Down List
button and select General
Demographic from the list.
Click the New Request link. The General Demographic Request page displays your current personnel information.
The message displayed in the General Information area is entered by the LINQ EP administrator. This message may change so it is important to read the message each time you make a request. |
Make the necessary changes to the data entry fields. Click here for a description of each field.
In the Comments area, type comments to the approver about this request. You can enter a maximum of 2,000 alphanumeric characters.
Click to select one of the following options:
E-mail me each step of the approval process radio button to receive an e-mail notification each time the request is approved.
E-mail me only on the final step of the approval process radio button to receive an e-mail notification only when your request receives final approval.
Do not e-mail me about this request radio button to not receive any e-mail notification about the approval status of your request.
Click the
button to submit your request. If the E-mail
me each step of the approval process radio button is selected,
you will receive an automated e-mail that contains the request details
and its current status (pending).
The request is automatically routed to the first approver in the approval chain.
